OP NOTE: “We were able to open the right atrium and see that the right ventricular mass was deep down into the RV. It was well encapsulated. All of the chords were attached from the anterior and septal leaflets of the tricuspid valve to this large mass. It was carved out, removing it from the chords of the leaflets of the tricuspid valve and also then from the attachments alongside the entire papillary muscles of the free wall of the right ventricle. This was the size of a golf ball. This was resected in total without entering it.
After this was done, we then were able to go ahead and see that the tricuspid valve had been more or less destroyed…”
PATH: Mature Cystic Teratoma (contains multiple cysts lined by respiratory or mucinous epithelium, cartilage, salivary gland tissue and small foci of brain tissue. Neither immature nor malignant elements are identified)
Extremely rare in adults. Second most common primary cardiac masses in fetus and neonates (after rabdo) and when present 90% are pericardial. The mass in this case clearly did not read the textbook.
UPDATE from Yoel: The case presented during the last meeting with a question of peri-partum cardiomyopathy was lost for follow up. We may never know…
